We are seeking a highly skilled Registered Nurse to join our team at St. Albans Health and Rehab Center in Saint Albans, VT. As a Care Management Registered Nurse, you will play a vital role in providing high-quality patient care and collaborating with the nursing team to develop effective plans of care.
Key Responsibilities:- Perform comprehensive nursing assessments and collaborate with the interdisciplinary team to develop individualized care plans.
- Establish realistic, measurable patient goals and interventions to minimize rehospitalizations and optimize patient outcomes.
- Delegate patient care responsibilities to staff and supervise them to ensure that tasks and interventions are implemented effectively.
- Communicate patient information with assigned staff and between shifts to ensure seamless care delivery.
- Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- CPR Certification is required.
- Competitive hourly rate of $44.00 per hour.
