Nurse Care Specialist

BAYADA Home Health Care is seeking a compassionate and dedicated Registered Nurse (RN) to join our team in South Burlington, VT.
This role involves providing high-quality patient care to adults and geriatric clients in their homes, with a focus on comfort, independence, and dignity.
Responsibilities:- Follow a designated care plan to meet patient needs
- Make home visits to clients in designated territories
- Perform assigned duties, including medication administration, wound care, and treatments
- Monitor patient conditions and report changes to Clinical or Client Services Manager
- Document observations, interventions, and evaluations using a state-of-the-art touch pad tablet
- Current license as a Registered Nurse in Vermont
- Graduation from an accredited nursing program
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred
