Compensation Details
- Salary range is between $120,000 and $150,000, depending on experience.
Position Highlights
- This role requires full-time availability, with the possibility of weekend shifts if preferred.
- Flexible scheduling options may include 4/10 hour shifts instead of the traditional 5/8 hour shifts.
Key Responsibilities
- Conduct annual wellness evaluations and health assessments for patients with chronic illnesses.
- Provide educational resources to enhance patient health.
- Assist in addressing quality care gaps, including necessary screenings and laboratory tests.
- Collaborate effectively to improve care delivery.
Qualifications
- Must possess a valid, unencumbered Nurse Practitioner License for the relevant state.
- Experience in in-home risk assessments is advantageous but not mandatory.
- A minimum of 3 years of patient care experience is required, preferably in primary care, adult, or geriatric settings, along with EMR proficiency.
- Bilingual candidates are encouraged to apply.
- A High School Diploma or GED is necessary.
- Mandatory attendance is required for the initial training period.
