Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team at Meridian Health Services. As a key member of our Primary Care Office, you will play a vital role in providing high-quality patient care and contributing to our mission of delivering whole-person healthcare.

Key Responsibilities:

  • Respond to patient concerns regarding medication effectiveness, side effects, and special precautions, consulting with medical staff as needed.
  • Phone in prescription orders to pharmacies as directed by medical staff.
  • Review lab and test results, reporting concerns and abnormal results to medical staff.
  • Obtain medical histories and perform physical assessments as directed by medical staff, including blood pressure, weight, and AIMS testing.
  • Administer injectable medications as ordered by medical staff.
  • Complete documentation to ensure accurate and timely information for client and third-party billing.

Requirements:

  • Licensed Practical Nurse (LPN) in the state of Indiana required.
  • One (1) year experience in Nursing required.
  • Experience in substance use/psychiatric nursing experience preferred.
  • A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
