SunCrest Home Health is seeking a skilled Physical Therapist to join our team of healthcare professionals. As a Physical Therapist, you will play a vital role in providing high-quality patient care and rehabilitation services to our clients in the comfort of their own homes.
Key Responsibilities- Assessment and Evaluation: Conduct thorough assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain, and other medical conditions.
- Treatment Planning: Develop and implement individualized treatment plans to improve and enhance patient well-being.
- Interventions and Progress Monitoring: Perform interventions aimed at improving patient function and mobility, and evaluate patient progress.
- Communication and Collaboration: Communicate effectively with patients, families, caregivers, and other healthcare professionals to ensure seamless care coordination.
- Documentation and Reporting: Maintain accurate and detailed records of patient progress, treatment plans, and outcomes.
- Licensure: Current Physical Therapy licensure in the state of practice.
- Certification: Current CPR certification.
- Transportation: Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
