About the Role

As a Home Health Registered Nurse with CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Our team is dedicated to providing comprehensive care that addresses the physical, emotional, and social needs of our patients.

Key Responsibilities
  • Provide skilled nursing visits to patients in their homes, assessing their needs and developing personalized care plans.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination.
  • Conduct thorough assessments, develop and implement care plans, and provide ongoing evaluation and revision of patient care.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to care plans.
  • Supervise and coordinate the work of home health aides and other support staff to ensure high-quality care delivery.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum one year of nursing experience, preferably in home health or a related field.
  • Strong med-surg, ICU, ER, and acute care experience.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.
