Job Overview

AdvisaCare Hospice and Palliative Care Program seeks a skilled Nurse Practitioner to join our interdisciplinary team. As a key member of our care team, you will provide high-quality patient care, collaborate with healthcare professionals, and contribute to the development of our palliative care program.

Key Responsibilities:

  • Conduct comprehensive patient assessments, develop and implement individualized care plans, and coordinate with healthcare teams to ensure seamless care transitions.
  • Collaborate with Medical Director, attending Physician, and Interdisciplinary Team to identify patient needs and develop effective care strategies.
  • Provide education and support to patients and families on disease management, symptom control, and advance care planning.
  • Participate in Quality Assurance activities, committee meetings, and professional development opportunities to enhance clinical skills and stay current with industry best practices.

Requirements:

  • Current unrestricted Nurse Practitioner license in Michigan.
  • Minimum one-year experience in a Medicare-certified home health/hospice agency preferred.
  • DEA Certification for prescribing controlled substances and ability to maintain confidentiality.
  • Excellent interpersonal and communication skills, with ability to work effectively in a team environment.
