Job Summary:
We are seeking a compassionate and dedicated Hospice Registered Nurse to provide exceptional end-of-life support to our terminally ill patients. As a key member of our hospice care team, you will be responsible for assessing patients' medical conditions, maintaining their comfort, and providing guidance and support to their loved ones.
Responsibilities:
* Collaborate with physicians, social workers, chaplains, and nursing assistants to develop and implement patient care plans
* Prepare and adjust patient care plans as needed
* Visit patients frequently to monitor vital signs and physical comfort levels
* Perform prescribed treatments and administer pain management medications
* Provide loved ones and primary caregivers with patient care information, end-of-life education, and emotional support
* Maintain and update medical charts, patient files, and health records
* Notify attending physicians, hospice staff members, and family members of patients' deaths when they occur
* Advise grieving family members on coping techniques, counseling services, and available community resources

Requirements:
* Active Registered Nurse (RN) license in state of practice
* Bachelor of Science degree in nursing (BSN) preferred
* Accredited certification in hospice and palliative care required
* Two (2) years' experience as a hospice/Intensive Care Unit or Long Term Care nurse preferred
* In-depth knowledge of monitoring and managing terminally ill patients' conditions and comfort
* Superb analytical and diagnostic skills
* Great interpersonal, communication, and collaboration abilities, as well as a compassionate nature
* Exceptional ability to manage stress and emotional challenges
