Key Responsibilities:
  • Collaborate with physicians, directors, and other healthcare providers to identify and implement innovative models and best practices, emphasizing service improvements and cost reduction.
  • Develop and implement quality and utilization standards across the continuum of care to ensure coordinated plans of treatment, customer-focused delivery of services, and cost-effective utilization of necessary services.
  • Ensure compliance with administrative, legal, and regulatory requirements of the Health Plan contract and governmental and accrediting agencies.
  • Develop and enhance unit-based standards as identified by regulatory agencies.
  • Develop a service line that achieves a high level of customer satisfaction, with emphasis on customer service and highest standards of quality and innovation.
  • Assist with planning, development, education, implementation, monitoring, evaluation, and revision of Infection Prevention and Control Program goals, standards, and policies and procedures, including meeting applicable regulatory requirements.
  • Coordinate multi-disciplinary and interdepartmental efforts to improve performance of infection prevention and control programs and services.
  • Maintain ongoing communications and cooperative relationships with medical center committees and external agencies, including but not limited to Infection Prevention and Control leadership.
  • Participate in epidemiological investigations of significant clusters of infection above established standards as directed by the Infection Prevention and Control leadership.
  • Conduct infection prevention and control surveillance; collate, compile, analyze, and present reports as needed.
  • Provide infection prevention and control in-service education for orientation, annually, and the local service/medical center area as needed.
  • Utilize research data to implement clinical changes and the delivery of patient care and member services.
  • Manage and resolve human resource, labor relations, employee, and department safety and risk management issues.
  • Direct the budget and resource allocations.
  • Manage the financial performance and identify and implement strategies to reduce costs and improve quality of care and services.
Secondary Functions:
  • On-call coverage at least every 2-3 months.
Requirements:
  • Minimum of five (5) years of management experience, with three years in Home Health/Hospice.
  • Bachelor's degree in nursing or other related field.
  • Valid California driver's license.
  • Basic Life Support certification.
  • Demonstrate knowledge of professional clinical licenses/certifications, The Joint Commission, and other local, state, and federal regulations.
  • Knowledge of federal, state, and accreditation standards applicable to Home Health/Hospice required.
  • Knowledge of governmental and other regulator standards, requirements, and guidelines related to quality improvement.
  • Strong working knowledge of ongoing monitoring techniques (including criteria development and statistical analysis); care delivery in home care; total quality management principles, tools, and techniques.
  • Effective communication, negotiation, and leadership skills.
  • Demonstrated strong interpersonal communication skills.
  • Must be able to work in a Labor/Management Partnership environment.
Preferred Qualifications:
  • Masters degree preferred.
