BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team in Westampton, NJ. As a Physical Therapist, you will be responsible for providing high-quality patient care and rehabilitation services to clients in their homes.
Responsibilities:- Conduct home visits to assess and treat clients with physical therapy needs
- Develop and implement individualized care plans to promote client independence and mobility
- Perform diagnostic tests and measurements to evaluate client progress
- Implement evidence-based interventions to prevent wound formation and promote wound repair
- Document client observations, interventions, and evaluations in a state-of-the-art electronic medical record system
- Current New Jersey Physical Therapist license
- Minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem-solving skills
