About the Role

We are seeking a skilled Occupational Therapist to join our team at CenterWell Home Health. As a key member of our care team, you will work closely with patients, families, and healthcare professionals to provide high-quality, patient-centered care.

Responsibilities
  • Assess and develop individualized treatment plans to improve patients' functional abilities and quality of life.
  • Conduct thorough evaluations and assessments to identify patients' strengths, weaknesses, and goals.
  • Develop and implement effective treatment plans that address patients' physical, emotional, and social needs.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
  • Document patient progress and outcomes accurately and thoroughly.
Requirements
  • Master's degree in Occupational Therapy from an accredited program.
  • Current licensure as an Occupational Therapist in the state of practice.
  • Minimum of six months of experience in occupational therapy, preferably in home health.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.
