About the Job

We are seeking a skilled Occupational Therapist to join our team at CenterWell Home Health. As a Home Health Occupational Therapist, you will play a vital role in helping patients recover from illness, injury, or surgery and regain their independence.

Key Responsibilities
  • Assess and screen patients' daily living and work-related skills to develop personalized therapeutic retraining programs.
  • Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patients' level of function.
  • Collaborate with healthcare teams to develop and revise treatment plans, ensuring patients receive comprehensive care.
  • Provide therapeutic treatment and instruction to patients, improving strength, coordination, range-of-motion, and function, or teaching compensation measures.
  • Review and expand retraining programs to align with patients' behavioral goals and rehabilitation program objectives.
  • Consult with vocational team members to develop vocationally oriented plans, promoting patients' independence and community reintegration.
  • Communicate patient status and progress to physicians, rehabilitation staff, Clinical Managers, patients, and family members.
  • Participate in care coordination and discharge planning activities, ensuring seamless transitions and optimal patient outcomes.
  • Design community reintegration activities to support patients' physical reconditioning and psychological adjustment.
  • Recommend and design special adaptive equipment to enhance patients' living and working environments.
  • Train staff and family members to carry out retraining programs, promoting patients' independence and self-care skills.
  • Supervise and evaluate Occupational Therapy Assistant and Home Health Aide performance, ensuring high-quality care delivery.
  • Accurately document patients' care observations, interventions, and evaluations, ensuring timely and thorough reporting.
Requirements
  • Master's degree from an accredited Occupational Therapy Program.
  • Minimum six months of occupational therapy experience, with Home Health experience preferred.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
