Inpatient Social Work Case Manager Position

Direct Recruit Agency is seeking a highly motivated and experienced Inpatient Social Work Case Manager to join our team. This is a full-time, permanent position with a competitive salary and benefits package.
Key Responsibilities:- Conduct thorough assessments of patients' psychosocial needs and develop individualized treatment plans.
- Collaborate with the healthcare team to ensure effective communication and coordination in patient care.
- Provide counseling and support to patients and their families, addressing emotional, social, and financial concerns.
- Advocate for patients' needs and assist with navigating the healthcare system.
- Monitor and evaluate patients' progress, adjusting treatment plans as needed.
- Coordinate with community resources to provide patients with appropriate support services.
- Maintain accurate and timely documentation of all case management activities.
- Bachelor's or Master's degree in Social Work from an accredited institution.
- Current state licensure as a Social Worker.
- Minimum of 2 years of experience in a healthcare setting, preferably in an inpatient setting.
- Strong understanding of psychosocial issues and ability to provide effective counseling and support.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Knowledge of community resources and ability to coordinate care with other providers.
