We are seeking a skilled Inpatient Social Work Case Manager to join our team at LifeBridge Health. As a key member of our healthcare team, you will provide patient and family advocacy, discharge planning coordination, and psychosocial intervention for high-risk inpatients.
Key Responsibilities:
- Collaborate with the clinical team and medical provider to promote patient and family wellness
- Coordinate discharge planning to ensure seamless transitions to community resources
- Provide psychosocial intervention to address complex health needs
- Develop and implement individualized care plans to improve patient outcomes
Requirements:
- Master's degree in Social Work (MSW) required
- 2+ years of hospital social work or community social services experience preferred
- LMSW required; LCSW/LCSW-C preferred
- 3-5 years of experience in a similar role
