Key Responsibilities

Position Summary

The Lead Case Manager plays a pivotal role in guiding the case management team by offering training, support, and structure for participants. This position involves developing program plans, managing participant caseloads, and implementing policies as directed by the relevant agency.

Primary Duties Include:

  • Leading the activities of the case management team, providing assistance with complex inquiries, and training new staff.
  • Managing a caseload using evidence-based practices; conducting assessments and creating individualized plans for each participant.
  • Preparing tailored programs for participants, establishing measurable goals, and reviewing progress regularly.
  • Orienting new participants to the program and ensuring they understand their responsibilities.
  • Maintaining confidential participant files and ensuring compliance with privacy standards.
  • Participating in team meetings to share insights and collaborate on solutions.
  • Upholding confidentiality regarding sensitive information.
  • Reporting any program violations to the appropriate authorities.
  • Conducting orientation sessions to explain program rules and regulations.
  • Adhering to company policies and safety regulations.
  • Communicating effectively with all staff levels to foster teamwork.
  • Performing additional duties as assigned.
Qualifications

Minimum Requirements:

  • High School Diploma or equivalent; a Bachelor's degree in a Human Services discipline is preferred.
  • One year of experience as a Case Manager or three years in a related field.
  • Certification in substance abuse or domestic violence is preferred.
  • Bilingual in English/Spanish is advantageous but not mandatory.
  • Ability to engage professionally in individual and group settings.
  • Strong data collection and analysis skills.
  • Experience in leading a team and implementing strategic approaches.
  • Excellent organizational skills and attention to detail.
  • Proficient in Microsoft Office Suite, including Word, Excel, and Access.
