We are seeking a compassionate and motivated Registered Nurse - RN with a wound focus to integrate and lead a patient-centered team approach for skin health and wound care as a Wound Care Coordinator - RN at Lebanon Center - Lebanon, NH.
Key Responsibilities- Lead and integrate a fully developed Skin Health and Wound management program in collaboration with nursing center management.
- Oversee durable medical equipment related to the prevention and management of wounds.
- Develop and implement wound care protocols and procedures to ensure high-quality patient care.
- Collaborate with interdisciplinary teams to provide comprehensive wound care services.
- Conduct wound assessments and develop individualized care plans for patients with complex wounds.
- Provide education and training to nursing staff on wound care best practices and protocols.
- Participate in quality improvement initiatives to enhance wound care outcomes.
- Must possess a RN license (MPT/DPT with leadership review/approval)
- Must complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire
- Commitment to attend monthly support calls is required
- Must have national accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire
- Knowledge and experience with clinical charting, incident reporting and investigation response is required
- Must have a strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team
- Experience in team building and demonstrating respect for the interprofessional team in wound management is required
- Knowledge of nursing home regulations and survey process is required
- Strong Microsoft Word, Excel, Database Management, PowerPoint skills are required
- Competitive salary range: $95,000 - $104,000 per year
- Full-time position with a Monday-Friday dayshift schedule (as part of a nursing center team, occasional evening/weekend hours may be required)
