Job Title: Speech Therapist for Home Health
Job Summary:
VNA Health Group is seeking a skilled Speech Therapist to join our team in providing high-quality speech and language therapy services to patients in their homes or alternate community settings within sections of Essex County, NJ.
Responsibilities:
- Assess and evaluate clients for speech and language therapy needs and develop and implement appropriate plans of action in collaboration with physicians.
- Document all changes in clients' conditions and update care plans and outcomes accordingly.
- Identify counseling needs and plan appropriate teaching strategies.
- Monitor quality and appropriateness of services provided.
- Adhere to agency infection control and safety policies.
- Adhere to the organization's policy in regards to absenteeism and appearance.
Qualifications:
- Licensed Speech-Language Pathologist with CCC-SLP certification and Master's degree in Speech-Language Pathology.
- 2-3 years of prior clinical experience and community-based experience preferred.
- Strong computer skills.
- Strong communication and interpersonal skills.
- Valid NJ driver's license.
Working Conditions/Physical Demands:
- Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation.
