We are seeking a highly skilled Licensed Practical Nurse to join our team at the Veterans Affairs, Veterans Health Administration. As a Licensed Practical Nurse, you will provide high-quality patient care in our Extended Care Service, working closely with our interdisciplinary team to deliver exceptional care to our patients.
Responsibilities- Provide comprehensive and thorough nursing care to patients with a variety of physical and/or behavioral problems.
- Function as a leader or co-leader for a specifically identified group of patients, demonstrating independent technical judgment and a detailed working knowledge of complex practical nursing skills and procedures.
- Administer medications via all routes, excluding intravenous, and complete pill box refill and medication reconciliation while in the home.
- Educate patients and/or family members for understanding of orders as documented in CPRS and carry out complex treatments as ordered, monitoring patients' response to medication and treatments.
- Report changes to providers and RNs and document effectiveness.
- Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC).
- Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
- Must be proficient in written and spoken English.
This position requires the ability to work in a fast-paced environment, with frequent interruptions and changing priorities. The successful candidate must be able to lift 45 pounds or more, stand for long periods, and walk for extended periods. The position also requires the ability to work in a team environment and communicate effectively with patients, families, and healthcare team members.
