The Licensed Practical Nurse (LPN) is responsible for delivering high-quality patient care through nursing interventions and supporting the healthcare team in an acute rehabilitation setting.

Key Responsibilities:

  • Deliver direct nursing care and support patient care under the guidance of Registered Nurses or other healthcare professionals
  • Execute the care plan by providing basic nursing care within the LPN scope of practice
  • Conduct phlebotomy services and administer prescribed medications and treatments as directed by healthcare practitioners
  • Contribute to the care plan with insights from patients, families, and other healthcare team members
  • Perform other assigned responsibilities

Requirements:

  • High School Degree or GED
  • Completion of an accredited LPN program
  • Current state licensure as an LPN
  • Basic Life Support (BLS) certification (preferred)
  • 1 year of clinical patient care experience (preferred)
  • 1 year of experience in an acute rehabilitation facility (preferred)
  • EPIC Electronic Health Record (EHR) training (preferred)
