Job Title: Senior Case Manager RN

About the Role

We are seeking a highly skilled and experienced Senior Case Manager RN to join our team at Professional Case Management. As a Senior Case Manager RN, you will be responsible for providing direct care and case management to assigned clients, while also supervising and mentoring junior staff members.

Key Responsibilities

  • Provide direct care and case management to assigned clients, including assessing client needs and developing individualized care plans.
  • Supervise and mentor junior staff members, including RN Case Managers and other clinical personnel.
  • Collaborate with regional directors and other personnel to identify and correct issues and/or improve services.
  • Participate in interviewing, selection, and ongoing evaluation of clinical personnel as requested by the Regional Director.
  • Perform other functions as requested by the regional director, including but not limited to personnel training, education, and competency validation.

Requirements

  • Graduate of a state-approved school of professional registered nursing.
  • BSN preferred.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
  • One (1) year of supervisory and/or case management experience preferred.
  • Current CPR certification.
