We are seeking a skilled and compassionate Home Health Nurse to join our team at the University of Virginia Health System. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.
Key Responsibilities:- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
- Develop and implement comprehensive care plans, working closely with patients, families, and healthcare providers to ensure seamless transitions and optimal outcomes.
- Provide direct patient care, including wound management, medication administration, and education on self-care and disease management.
- Collaborate with the interdisciplinary team to coordinate care, communicate effectively, and ensure timely interventions.
- Participate in performance improvement activities, contributing to the development and implementation of evidence-based practices and quality initiatives.
- Graduate of an accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire.
- 1 year of relevant experience in home health or a related field.
- Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia.
- American Heart Association (AHA) Health Care Provider BLS certification required.
