About the Position

We are seeking an experienced Registered Nurse (RN) to lead our home health team as a Clinical Lead. In this role, you will be responsible for overseeing the delivery of high-quality care to our patients in their homes, working closely with other healthcare professionals to develop and implement individualized care plans.

Responsibilities
  • Supervise and manage a team of RNs and unlicensed caregivers to ensure effective care delivery
  • Conduct regular home visits to assess patient needs and evaluate the effectiveness of care plans
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless transitions and continuity of care
  • Maintain accurate and up-to-date records of patient progress and care plans
Salary Information

The estimated salary range for this position is $65,000-$85,000 per year, based on factors such as location, experience, and qualifications. This figure does not include additional compensation opportunities that may be available, such as shift differentials, bonuses, or overtime pay.

Requirements
  • Current Oregon Registered Nurse License
  • National Provider BLS Certification from the American Heart Association
  • Relevant certifications applicable to the position, as specified by the Nurse Manager
