Job Title: Admissions Registered Nurse

At Interim HealthCare of the Twin Cities, we are seeking a skilled and compassionate Admissions Registered Nurse to join our team. As a key member of our Home Care team, you will be responsible for completing admissions and visits for our patients, providing high-quality care and support in their home environment.

Key Responsibilities:
  • Conduct thorough assessments and develop personalized care plans for patients
  • Provide skilled nursing care and support to patients in their homes
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Document patient progress and outcomes accurately and timely
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements:
  • Graduate of an accredited school of nursing and licensed to practice as a Registered Nurse in the state of Minnesota
  • Minimum of 12 months experience working in an acute care or home health environment
  • Evidence of current CPR certification
  • Experience in Home Care, including OASIS, preferred
  • Able to travel within a geographic area serviced by the office, with reliable transportation and valid driver's license
