Job Description:

As a Home Health Physical Therapist with AccentCare, you will have the opportunity to work with patients in their homes, helping them to recover from illness or injury and maintain their independence. Your expertise and compassion will be instrumental in improving the quality of life for our patients and their loved ones.

Key Responsibilities:

* Conduct comprehensive assessments of patients' physical abilities and develop personalized plans to address their needs
* Deliver evidence-based interventions to improve patients' mobility, strength, and function
* Collaborate with other healthcare professionals to ensure seamless coordination of care

Qualifications:

To be successful as a Home Health Physical Therapist with AccentCare, you will need:

* A Master's degree in physical therapy from an accredited program
* Current licensure to practice as a physical therapist in the state where you will be working
* At least 9 months of experience as a physical therapist, with a preference for those with community or home health experience
