Nurse (Outpatient) - Clinical Care Specialist

The US Department of Health and Human Services is seeking a skilled Nurse (Outpatient) to provide comprehensive care to pediatric, adolescent, adult, and geriatric patients within the Great Plains Area. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care, assessing patient needs, and developing individualized care plans.
Key Responsibilities- Provide direct professional nursing care to patients, reviewing medical history, assessing current health conditions, and evaluating patient self-care abilities.
- Administer medication in accordance with the plan of care and assist providers with procedures.
- Participate in patient-centered medical home by providing care coordination based on patient needs, assisting patients in establishing goals, and providing education to patients and families to improve health outcomes.
- Maintain certifications in Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), and other relevant certifications.
- Collaborate with healthcare providers to develop and implement individualized care plans, ensuring seamless patient care.
- At least one year of specialized experience equivalent to the next lower grade level in the Federal service, performing tasks such as providing direct professional nursing care, administering medication, and participating in patient-centered medical home.
- Possession of a bachelor's degree in Nursing or a diploma/associate degree in professional nursing and relevant experience.
- Current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
- Ability to work effectively in a team environment, communicate effectively with patients and healthcare providers, and maintain confidentiality and professionalism.
This position requires the ability to work in a fast-paced environment, handle multiple priorities, and maintain a high level of professionalism and confidentiality. The selected individual must be able to pass a pre-employment fingerprint check and background investigation, and maintain medical staff clinical privileges, including any licensure requirements.
