Job Description

We are seeking a skilled Clinical Program Coordinator to join our team at Horizon House, ensuring that individuals receive the necessary services to thrive in their local community. This role involves identifying and providing regular supports to clients, as well as maintaining continuity of care.

Key Responsibilities:

  • Service Delivery: Ensure compliance with contract and agency regulations, guaranteeing a continuous flow of services for clients.
  • Service Design: Collaborate in developing and refining individual rehabilitation plans, incorporating social, educational, vocational, self-care, and residential skills and objectives.
  • Gaps Analysis: Assess and identify gaps in service delivery, recommending alternative action plans to ensure seamless support.
  • Crisis Intervention: Participate in resolving crisis situations, assisting with identifying key individuals and resources for direct support and/or immediate intervention.
  • Documentation: Submit accurate documentation and related information on a timely basis, adhering to funding source regulations.

Qualifications:

  • Bachelor's degree in Human Services, Social Science, or Healthcare required.
  • Three years of Mental Health direct care experience; or any combination of experience, education, or training providing equivalent knowledge, skill, and ability.
  • Must pass a background check.

The estimated salary for this role is $65,000 - $85,000 per annum, depending on location and experience. We offer a competitive benefits package, including health insurance, retirement plan, and paid time off.
