Job SummaryWe seek a Nurse Practitioner or Physician Assistant to join our outpatient practice in Lawrenceville, Georgia. This role will involve working closely with the medical director to deliver high-quality patient care and contribute to ongoing practice innovation.
Key Responsibilities- Provide comprehensive primary care in an outpatient setting
- Partner with the care team to engage geriatric patient populations in care and drive behavior change
- Leverage coaching and integrated behavioral health models to maximize impact on the population
- Utilize our proprietary Electronic Health Record to plan care for individual patients
- At least 2 years of primary care experience required
- Currently licensed or ability to obtain licensure in the state of Georgia
- Ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups
