About the Role
">CenterWell Senior Primary Care is seeking a skilled and compassionate Primary Care Physician to join our team. As a valued member of our healthcare community, you will play a vital role in delivering high-quality, patient-centered care to seniors. With a focus on proactive, preventive care, you will work closely with our integrated care teams to provide comprehensive services, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment, and more.

- Evaluate and treat patients in accordance with standards of care, working collaboratively with our care teams to ensure seamless coordination of services.">
- Act as an active participant and key source of medical expertise with the care team through daily huddles, helping to set a tone of cooperation and professionalism.">
- Assist in the coordination of patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.">
- Participate in quality improvement initiatives, working to optimize patient outcomes and meet the needs of our growing population.">
Requirements
">- ">
- Current and unrestricted medical license, or willingness to obtain a medical license in the state of practice.">
- Graduate of an accredited MD or DO program from an accredited university.">
- Excellent verbal and written communication skills, with the ability to effectively collaborate with colleagues and patients.">
- Demonstrated commitment to the concept of integrated team-based care and a willingness to learn and adapt to a value-based care setting.">
- Superior patient/customer service skills, with the ability to prioritize patient needs and deliver exceptional care.">
Preferred Qualifications
">- ">
- Board Certification or Eligibility to become certified (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine.">
- Active and unrestricted DEA license, Medicare Provider Number, and experience managing Medicare Advantage panels.">
- Knowledge of Medicare guidelines and coverage, HEDIS quality indicators, and a strong understanding of coordinated care environments.">

- Competitive salary: $199,400 - $298,300 per year, based on experience and qualifications.">
