Key Responsibilities
The Intake Coordination Specialist in Home Health is responsible for providing essential administrative support to the Central Intake Teams by effectively managing incoming calls and processing referrals within our operating system. Key duties include:
- Receiving and accurately relaying referral information within the Central Intake and to various branches.
- Answering incoming calls and appropriately directing them to the Central Intake Teams.
- Communicating patient and referral details to the relevant Central Intake Staff and branches.
- Gathering demographic and clinical data from clients, physician offices, and other referral sources to compile comprehensive referrals.
- Facilitating communication of referral information with Central Intake Nurses.
- Transferring specialized referral calls to the appropriate nursing staff.
- Managing incoming calls and messages from referral sources, ensuring effective communication.
- Providing general information about agency services to patients, families, and referral sources as needed.
- Entering Central Intake and branch referrals into the mainframe system as required.
- Participating in workshops, continuing education programs, and annual training to stay updated on departmental changes.
- Managing patient data feeds throughout the day in applicable markets.
- Distributing patient data to relevant personnel in applicable markets.
- Performing additional duties as assigned.
Qualifications
Applicants should possess a minimum of a High School diploma or G.E.D., along with one to two years of experience in office procedures and insurance/medical settings.
