Job Title: Physical Therapist

LHC Group is seeking a skilled Physical Therapist to join our team in Belmont, Massachusetts.

Job Summary

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our patients in their homes. As a Physical Therapist with LHC Group, you will have the opportunity to work with a diverse range of patients and develop a personalized treatment plan to meet their unique needs.

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, and develop a treatment plan to improve their mobility and overall well-being.
  • Provide one-on-one therapy sessions to patients in their homes, using a variety of techniques and modalities to promote healing and recovery.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordinate with patients' families and caregivers to ensure seamless care transitions.
  • Document patient progress and outcomes, and communicate with the healthcare team to ensure that patients receive the best possible care.
Requirements
  • Current Physical Therapy licensure in the state of Massachusetts.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
