About the Opportunity

An exciting chance to work in an adult primary care practice serving College employees and their dependents in New Hampshire.

  • 32 patient contact hours per week
  • Average daily patient census is 8-10 with 30 and 60-minute appointments
  • No call
  • Collaborative team of 2 Physicians, a health coach, and care team manager
  • On-site behavioral health services
Key Responsibilities

Manage a patient panel with diverse needs, conduct a mix of acute, chronic, and well visits, and treat patients in-office or testing centers. Occasional telehealth visits may be required. Ongoing collaboration with teammates via daily huddles and virtual clinical teams.

Requirements

A passion for human-centered primary care, ability to effectively use technology, clinical proficiency in evidence-based primary care, and a desire to be part of a team dedicated to changing healthcare delivery. Ability to navigate uncertain situations and adapt to meet patient needs.
