Job Summary

Milestone Inc. is seeking a skilled Licensed Practical Nurse to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality care to individuals with diverse needs.

Key Responsibilities
  • Coordinate and perform nursing duties and procedures to ensure the comfort and well-being of individuals.
  • Establish and maintain effective communication with individuals, parents, and guardians, communicating medication changes and health status.
  • Function as a member of the IDT and recommend health/medical goals for individuals.
  • Implement individual health care plans as approved by IDT and Human Rights Team.
  • Administer prescribed medications and treatments, observing and documenting individual reactions.
  • Keep individual medical records current on nursing observations and treatments rendered.
  • Coordinate specimen/culture collection and transportation, and record results.
  • Assist with admission, discharge, and transfer of individuals.
  • Demonstrate knowledge of DCFS Licensing Standards and provide guidance to staff to ensure compliance.
Requirements
  • Current State of Illinois License in good standing, Licensed Practical Nurse.
  • High School Diploma or G.E.D.
  • Ability to intervene physically in behavioral and health-related crisis with individuals.
  • Current in CPR.
  • Ability to respond promptly to emergency situations.
  • Must be fingerprinted and pass background check in accordance with DCFS standards.
