Key Responsibilities:- Interview, educate, train, and supervise clinical team members to ensure high-quality patient care.
- Triage clients, including admissions, recertifications, and discharges, to ensure seamless care transitions.
- Collaborate with clients, the Clinical Director, and other care team members to develop individualized Plans of Care that meet each patient's unique needs.
- Review documentation submissions for accuracy and quality, ensuring uninterrupted authorization for services.
- Conduct home and/or hospital visits to assess patient needs and provide guidance to clinical team members.
