Senior Community Care Opportunity

Optum is seeking a skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Portland, OR. As a member of our interdisciplinary team, you will provide high-quality care to patients in their homes, nursing homes, and assisted living facilities.

Key Responsibilities:
  • Conduct comprehensive assessments and develop individualized care plans
  • Manage patients through acute medical situations and chronic condition exacerbations
  • Collaborate with the interdisciplinary care team to ensure seamless care transitions
  • Provide education and counseling to patients and caregivers
  • Document patient information accurately and efficiently

Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (for Nurse Practitioners) or accredited Physician Assistant degree program (for Physician Assistants)
  • Active and unrestricted license in the state of practice
  • Board certification in a relevant specialty (e.g., Family Nurse Practitioner, Adult-Gerontology Acute Care Nurse Practitioner)
  • Ability to work independently and collaboratively as part of an interdisciplinary team
Preferred Qualifications:
  • 1+ years of clinical experience in senior living, geriatric medicine, or home care
  • Experience working with patients with complex behavioral, social, and functional needs
  • Understanding of geriatrics and chronic illness management
