We are seeking a skilled Oncology Navigator to join our team at SSM Health. As a key member of our healthcare team, you will provide exceptional patient care and support throughout the continuum of cancer treatment.
Responsibilities- Collaborate with physicians and advanced practice providers to ensure quality outcomes and coordinate treatment plans.
- Provide patient education and support to promote informed decision-making and improve health outcomes.
- Compile data and track outcomes to identify areas for process improvement and quality enhancement.
- Develop and maintain relationships with community organizations and healthcare providers to ensure seamless care coordination.
- Utilize the nursing process to assess, plan, intervene, and evaluate patient care.
- Graduate of an accredited school of nursing or equivalent education.
- Three years of registered nurse experience, preferably in oncology or a related field.
- Basic Life Support (BLS) certification and a valid nursing license in the state of employment.
