Job Summary

We are seeking a highly skilled and experienced RN Clinical Care Coordinator to join our team at Northwest Medical Center. As a key member of our Stroke Program, you will play a vital role in ensuring the highest quality patient care and outcomes.

Key Responsibilities
  • Oversee the daily operations and accreditation of the COE Stroke program, ensuring compliance with regulatory requirements and industry standards.
  • Coordinate and lead all activities for the Service Line program using an interdisciplinary approach, promoting efficient use of hospital resources and achieving operational and quality outcomes.
  • Assess and monitor the continuity of care for the defined patient population, expediting and coordinating the delivery of services to facilitate a patient's progress through the continuum of care.
  • Develop and update patient care protocols and procedures for Service Line patients based on current literature, trends, and evidence-based practice.
  • Act as a liaison between physicians and other healthcare professionals, ensuring effective communication and collaboration.
  • Perform data entry, retrieval, and reporting, as well as other administrative functions as necessary.
Requirements
  • Current Arizona RN license or compact state license.
  • American Heart Association (AHA) or American Red Cross (ARC) BLS certification.
  • Three (3) years experience in a clinical setting specific to the defined patient population, with experience in program development, third-party payers, and marketing preferred.
  • Strong analytical and organizational skills, with ability to compile statistics and perform data analysis.
  • Excellent human relations and oral/written communication skills, with prior computer software experience including word, excel, and graphics.
  • Spanish speaking preferred.
