About the Role

We are seeking a highly skilled and compassionate RN Patient Care Manager to join our team at Gentiva Hospice.

This is a safety-sensitive position that requires a strong commitment to delivering high-quality patient care and ensuring the provision of quality care to patients.

Key Responsibilities
  • Manage the day-to-day operations of your assigned hospice site, administering the clinical aspects of the hospice program.
  • Ensure the provision of quality care to patients, including managing and overseeing clinical and administrative functions.
  • Respond to phone referrals and inquiries in a timely, professional, and compassionate manner.
  • Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.
Requirements
  • Registered Nurse with a minimum of Associate's Degree in Nursing required from an accredited school of nursing.
  • Three years' experience in direct patient care in a clinical setting required.
  • Minimum one year of management/supervisory experience required.
  • Two or more years' experience in hospice or home care required.
  • Current license to practice nursing in the state of requested employment.
