Nurse Navigator

SSM Health seeks a skilled Nurse Navigator to support patient and provider needs on an inpatient and outpatient basis. This role will involve phone triage, care coordination, liaison between providers and patients/families, and education.
Responsibilities- Collaborate with physicians and advanced practice providers to ensure quality outcomes.
- Compile data, track outcomes, and make recommendations for process improvement.
- Provide referrals/support for follow-up care for patients who are uninsured or underinsured.
- Utilize the nursing process of assessment, planning, intervention, and evaluation in all patient encounters.
- Assist medical provider with processing of diagnostic results and bring significant values to the provider's attention.
- Recognize the implementation of dosages, interactions, side effects, adverse effects, routes of administration of drugs as they apply to individual patients.
- Establish working relationships teaching protocols for patient education, family counseling, and general public information.
- Graduate of accredited school of nursing.
- Three years' registered nurse experience.
Work in a constant state of alertness and safe manner. Perform other duties as assigned.
