Nurse Navigator

The University of Michigan Health-West is seeking a skilled Nurse Navigator to join their team. As a key member of the multidisciplinary team, the Nurse Navigator will serve as an advocate, liaison, educator, and support for patients and families throughout the care continuum.
The ideal candidate will have a strong background in nursing and experience in care management, with a focus on oncology. They will be responsible for ensuring patients receive quality and comprehensive services, including direct clinical practice, consultation, education, and research.
The Nurse Navigator will work closely with physicians, other Nurse Navigators, payers, and other members of the healthcare team to establish and promote a collaborative relationship. They will also assist in developing patient education programs and tools, and follow patients throughout the care continuum.
Requirements for the position include a current RN license in the State of Michigan, a Bachelor's of Science in Nursing, and three years of clinical nursing experience. The ideal candidate will also have knowledge of nursing principles, practices, and processes, as well as strong problem-solving, decision-making, and critical thinking skills.
Key Responsibilities:
- Assist patients in understanding their diagnosis, treatment options, and resources available
- Assess patient's ability to navigate the healthcare continuum and develop plan of care to overcome barriers
- Serve as liaison between patient and family, primary care physicians, internal and external care providers, specialists, and social services
- Facilitate appointments for consults and support services within established service standards
- Assist in developing patient education programs and tools
- Follow patients throughout the care continuum and collaborate with inpatient care management resources as needed
- Work with Registry staff and others to collect data, track outcomes, and support strategic planning processes
