Job Summary

The Home Living Specialist provides services for individuals with intellectual and developmental disabilities in their home or community, ensuring compliance with the Person Directed Plan (PDP). This role requires delivering Medicaid Waiver Home and Community-Based Services in accordance with the Evidentiary Certification Principles.

Key Responsibilities
  • Establish a relationship with individuals, providing assistance with daily living and social skills.
  • Ensure access to communication and information from the environment in clear and understandable methods.
  • Form a working alliance with family, neighbors, and professionals involved in the individual's life.
  • Participate in the development and implementation of the PDP.
  • Document areas identified as needs in the PDP.
  • Report accidents and existing dangerous conditions, observing safety practices.
  • Transport individuals to access community services and resources required by the plan of care.
  • Provide instruction and support in meal planning, self-care, medication management, household tasks, money management, and interpersonal communications.
  • Provide behavioral support and perform health-related tasks without standing physician orders.
  • Collaborate with the team to achieve goals, encouraging cooperation and commitment.
Qualifications
  • Able to recognize emergencies and implement emergency procedures.
  • Demonstrates respect for individuals, their property, and their privacy.
  • Ability to maintain confidentiality related to agency activities, personnel actions, and individual information.
  • Knowledge and use of universal precautions and basic infection control procedures.
  • Has on-the-job experience showing increased knowledge, skill, and abilities in providing services to more challenging individuals.
  • Ability to intervene with combative/aggressive individuals and perform physical training for Handle with Care physical intervention.
  • Ability to perform Cardiovascular Pulmonary Resuscitation (CPR).
  • High School Diploma or GED.
