Conemaugh Nason Medical Center is seeking a skilled Wound Care Nurse to join our team in the Wound Healing Program/Clinic. As a key member of our interdisciplinary team, you will coordinate clinical services to provide high-quality care for patients with wounds in inpatient and outpatient settings. Under the supervision of the Wound Care Director and Medical Director, you will work closely with physicians, healthcare professionals, and program staff to deliver exceptional patient care.

The ideal candidate will possess strong organizational, leadership, and planning skills, with the ability to work independently and as part of a team. You will be responsible for coordinating patient care, providing direct care, and overseeing the delivery of total quality care. Additionally, you will monitor utilization and outcomes, provide input on budget issues, and perform proper hand hygiene and follow infection control policies and procedures.

Essential Functions:

  • Coordinates patient care to ensure efficient and personalized care.
  • Initiates individualized patient and family teaching plans.
  • Makes decisions and judgments in preventing, analyzing, and resolving patient care concerns.
  • Effectively communicates patient status and anticipates situations that prevent/minimize adverse outcomes.
  • Continuously maintains and updates wound care knowledge with new and current treatment modalities.
  • Maintains proficiency in all aspects of documentation.
  • Participates in Performance/Process Improvement activities.
  • Demonstrates understanding and knowledge of patient's rights and responsibilities.
  • Collaborates with physician as part of the Interdisciplinary Team.
  • Demonstrates knowledge and understanding of wounds, treatments, interventions, and expected outcomes.
  • Oversees, provides, and coordinates delivery of patient care for wound patients.
  • Provides input on budget issues affecting efficient care delivery, judicious use of products, clinic organization, and staffing patterns.
  • Performs proper hand hygiene and follows all infection control policies and procedures.

  • Uses tact, sensitivity, sound judgment, and a professional attitude when relating with co-workers, patients, families, and visitors.
  • Performs duties in a self-directed and cooperative manner with minimal need for supervision.
  • Treats all co-workers, patients, families, and visitors with consideration to their rights, cultural diversity, privacy, comfort, and emotional needs.
  • Serves as a patient advocate and fosters a work culture of Diversity, Equity, and Inclusion.
  • Attends all mandatory trainings and completes all required Life Talent education assignments within a timely manner.
  • Keeps updated regarding Medical Center changes through meeting attendance and review of posted information.
  • Abides by and references the Employee Handbook and other pertinent facility policies, including clinical, non-clinical, safety, and infection control.

Job Requirements:

  • Graduate of a RN program required.
  • Bachelor of Science in Nursing preferred.
  • Current nurse's license in the state of Pennsylvania or a current multistate license (MLS) from compact member states/territories required.
  • Prior wound care experience.
  • Minimum three years' experience with direct hands-on care preferred.
  • Maintenance of current CPR Certification or BLS required.
  • Wound Care Certification preferred.
  • Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.
  • Possesses personal qualities which include kindness, understanding, respect, and/or ability to relate effectively to patients, families, and co-workers.
  • Basic computer skills.
