Care Manager Job Description
Blue Ridge Community Health Services is seeking a qualified Care Manager to join our team. As a Care Manager, you will play a vital role in ensuring the coordination and continuity of healthcare for our patients.
Key Responsibilities:
- Assess, plan, implement, coordinate, monitor, and evaluate the options and services required to promote quality, cost-effective health outcomes.
- Ensure the coordination and continuity of healthcare for patients as they transition from one facility to another.
- Oversee Care Management services and activities based on care management standards of practice for enrolled populations.
- Work with patients and caregivers to identify and address behavioral, social, cultural, and environmental strengths and barriers related to diagnosis, treatment, and access to care.
- Implement Care Management interventions, set goals, and develop the plan of care based on transitional care discharge plans/instructions, comprehensive assessments, and patients' goals.
- Facilitate and provide education to patients and families about clinical diagnosis, medications, available resources, prevention, and risk factors to achieve optimal self-management.
- Monitor quality and effectiveness of interventions to enrolled populations by setting patient-centered SMART goals in collaboration with patients' and families' identified goals.
- Delegate tasks and referrals to members of the care management team appropriately, accurately, and timely according to established workflows.
- Serve as an advocate and liaison among patients, families, community services, primary providers, prepaid health plans, specialists, and other care team members to coordinate services.
Requirements:
- Associate degree or higher from an accredited School of Nursing with an unrestricted license to practice nursing in NC or Licensed Clinical Social Worker (LCSW).
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Knowledge of care management standards of practice and principles of transitional care.
