Job Summary

Amergis is seeking a skilled LPN/LVN Tracheostomy School Nurse to provide high-quality patient care in a school setting. The ideal candidate will have experience in tracheostomy care and a strong understanding of the nursing process.

Key Responsibilities
  • Utilize the nursing process to plan, implement, and evaluate student care needs
  • Provide skilled nursing care in accordance with prescribed orders and Individualized Health Plans (IHP)
  • Perform interventions, treatments, and medication administration as outlined in the IHP
  • Monitor and document the effectiveness of interventions, treatments, and medications
  • Coordinate and facilitate services between the school, local health agencies, and other community resources to meet student needs
  • Communicate with school district personnel about student plans of care and IHP
  • Notify District School Nurse/Amergis Supervisor/Parent/Physician as outlined in the IHP regarding changes in student condition and physician notification for necessary orders
  • Collaborate with school/education personnel/other health agencies to design and meet student health care needs
  • Perform emergency tracheostomy changes as needed/required by student respiratory status
  • Provide tracheostomy care to students to maintain tube patency while minimizing risk for infection
  • Evaluate and assure student safety at all times
  • Adhere to Amergis and school district policies
  • Maintain accuracy in student health records
  • Participate in student care conferences/Individualized Education Program (IEP) as requested by the school district
Requirements
  • Licensed as a Licensed Practical/Licensed Vocational Nurse in state(s) where practicing
  • One year of School Nursing experience, preferred
  • At least one year of recent tracheostomy experience is required, unless approved by Amergis Clinical Designee
  • Completes tracheostomy competencies successfully
  • Complies with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program when applicable
  • Current CPR if applicable
  • TB Questionnaire, PPD or chest x-ray if applicable
  • Current Health certificate (per contract or state regulation)
  • Must meet all federal, state, and local requirements
  • Successful completion of new hire training as applicable to job site
  • Understand patient confidentiality and HIPAA requirements
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
  • Computer proficiency required
  • Must be at least 18 years of age
