Job Summary:

Sutter Health is seeking a skilled Home Health Nurse III/Telehealth Specialist to join our Advanced Illness Management Transitions team. As a key member of our team, you will be responsible for providing high-quality in-home nursing care and telehealth services to patients in the Central Valley, Roseville, and Modesto areas.

Responsibilities:
  • Assess, coordinate, and deliver in-home nursing care, including direct patient care and patient/family education.
  • Develop and implement individualized care plans that incorporate psychological, sociocultural, spiritual, economic, and lifestyle factors.
  • Foster collaborative relationships with patients, families, physicians, and other healthcare providers through timely and effective communication.
  • Adhere to policies, industry standards, best practices, and applicable laws and regulations to promote a quality patient experience.
Requirements:
  • Graduate of an accredited school of nursing.
  • RN-Registered Nurse of California.
  • BLS-Basic Life Support Healthcare Provider.
  • California Drivers License.
  • Valid Auto Insurance.
Preferred Experience:

5 years' experience working as an RN in a home health or ambulatory setting.

Skills and Knowledge:
  • Demonstrated proficient knowledge and competence with regard to nursing theories, concepts, and practices.
  • Working knowledge of the healthcare industry, safety precaution policies, and best practices regarding patient care and privacy.
  • Proficient knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record.
  • Time management and organizational skills, including the ability to prioritize patient care, assignments, and work within standardized policies, procedures, and practices.
