Covenant House California is seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing case management and custodial services to youth in our transitional housing program.
Job Summary:The Case Manager will work under the supervision of a Director to provide case management and custodial services to youth in our transitional housing program. This includes intake and assessment, case planning, documentation, and workshops. The Case Manager will also provide custodial supervision and ensure the cleanliness and safety of the facility.
Responsibilities:- Provide case management services to youth in our transitional housing program
- Conduct intake and assessment of youth, including reviewing intake documentation and ensuring it is entered into ETO and HMIS
- Develop and maintain Individual Service Plans (ISPs) with youth, including revising and updating the ISP as necessary
- Document regular progress notes in youth's case record (HMIS and ETO) for those youth assigned to his/her case load
- Facilitate psycho-educational and support group activities with youth on a regular basis
- Provide custodial supervision, including ensuring the cleanliness and safety of the facility, and providing crisis intervention as necessary
- Perform other program duties as assigned by the Director
- Bachelor's degree in Psychology, Sociology, Social Work, or related field
- OR Associate degree in Psychology, Sociology, Social Work, or related field and 2 years of case management experience, preferably with homeless or runaway youth
- Minimum of 4 years of case management experience, preferably with homeless or runaway youth, in lieu of a bachelor's degree
- Self-starter and team player with the ability to work with other staff
- Knowledge of adolescent development, community resources, and case management skills
- Good organizational skills and the ability to communicate with members, clients, vendors, and the public
- Bilingual in Spanish is preferred but not required
- Valid CA Driver's License, reliable transportation, and a good driving record
