Responsibilities- Implement and monitor patient care plans to ensure the highest level of care.
- Monitor and record patient condition, communicating effectively with the healthcare team.
- Coordinate patient care with other disciplines to ensure a well-coordinated care plan.
- Carry out physician and nursing orders to provide the best possible care.
- Assess and coordinate patient discharge planning needs with the healthcare team.
- Advanced Life Support certification within 6 months of hire date.
- BLS Provider certification within 1 month of hire date.
- Registered Nurse licensure from the Wisconsin Board of Nursing or current home state license.
