Job Title: LPN - Float (Full-Time)

Job Summary:

We are seeking a skilled Licensed Practical Nurse to join our team at Hudson Headwaters Health Network. As a Float LPN, you will provide high-quality patient care in a dynamic and fast-paced environment.

Key Responsibilities:

  • Provide age-appropriate patient care in a professional and compassionate manner.
  • Administer care to patients under the direction of a Registered Nurse or medical provider.
  • Collaborate with staff to develop strategies for improving quality of care and patient experience.
  • Utilize data to meet care coordination needs across settings.
  • Prepare patients for encounters by following established protocols.
  • Demonstrate competence in clinical skills and proper medication management.
  • Reconcile medication lists and prepare prescription renewals.
  • Document patient information accurately and timely in the electronic medical record.
  • Respond to patient phone calls and engage providers or RNs as needed.
  • Reinforce patient education plans to meet individual needs.
  • Perform point-of-care testing, including specimen collection and documentation.
  • Assist with patient flow and troubleshoot issues in collaboration with supervisors.
  • Provide guidance and mentorship to new and incumbent staff.
  • Attend meetings and serve on committees as assigned.
  • Assist with inventory and receipt of medical supplies, pharmaceuticals, and equipment.
  • Complete daily, weekly, or monthly tasks as assigned.

Requirements:

  • Unrestricted current LPN license to practice nursing in New York State.
  • Minimum of 1-year clinical experience preferred.
  • Ability to travel throughout the Network.
  • Current BLS certification.
  • Proficiency with Microsoft applications and electronic medical records.
  • Excellent verbal and written communication skills.
