Job Opportunity

Oakwood Village is seeking a skilled nursing professional to join our team as a Licensed Practical Nurse in our Skilled Nursing Facility. As a key member of our healthcare team, you will contribute to the assessment, planning, implementation, and evaluation of resident care on a defined unit.

Your responsibilities will include direct resident and family care, mentorship of Certified Nursing Assistant (CNA) staff, and collaboration with registered nurses and other interdisciplinary team members. You will work closely with nurses from all shifts and physicians to ensure the highest quality of care for our residents.

The LPN position reports to the Director of Resident Care/Director of Nursing and offers a range of shifts to accommodate your schedule. Oakwood Village is committed to providing a supportive and collaborative work environment, with opportunities for growth and development.

Responsibilities

  1. Perform resident care, including medication administration, completion of treatments, data collection, and basic teaching and reinforcement education.
  2. Contribute to the assessment, planning, implementation, evaluation, and documentation of resident conditions and care.
  3. Document and report changes in condition, intervention, and resident response in coordination with RN. Initiate/follow-up/discontinue acute care plans as appropriate.
  4. Update Care Plan consulting with RN as appropriate.
  5. Communicate Care Plan changes with Nursing and therapy staff.
  6. Attend Care conferences as designated.
  7. Report changes in resident condition to supervisor and seek guidance from RN as appropriate.
  8. Deliver nursing care in compliance with Nurse Practice Act, State and Federal regulations applicable to SNF setting and policies and procedures of Oakwood Village.
  9. Collaborate with nursing team, medical consultants, and other interdisciplinary team members to ensure resident and family needs are met.
  10. Demonstrate effective time management and problem-solving skills.
  11. Communicate with physicians regarding changes in condition, need for order clarification, and when necessary, transcription of physician orders, including EMR entry, per Oakwood policy.
  12. Other duties as assigned.
