Job Summary

We are seeking a skilled Occupational Therapist to join our team as a Home Health Occupational Therapist. As a key member of our team, you will work with patients in their homes to help them recover from illness or injury and achieve their maximum potential.

Responsibilities
  • Conduct thorough assessments of patients' physical and cognitive abilities to develop personalized treatment plans
  • Implement evidence-based rehabilitation techniques to improve patients' functional abilities and quality of life
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
  • Document patient progress and outcomes to inform treatment decisions and quality improvement initiatives
Requirements
  • Master's degree in Occupational Therapy or equivalent with licensure or certification within the state providing services
  • At least two years of experience working independently under minimal supervision
  • Must be a licensed driver who can travel to all business locations
