Occupational Therapist PRN Home Health Job Description

Advanced Home Health and Hospice of Arizona is seeking a PRN Occupational Therapist to join our Home Health team in the Phoenix area.

Job Summary

We are looking for a PRN (as needed) OT with pay varied depending on visit types. Our OTs plan and provide physical services to clients in their homes in accordance with the Plan of Care and Agency policies and procedures.

Responsibilities
  • Perform occupational therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care.
  • Provide assessment findings and goals to assist in developing the client care plan and obtain specific physician orders as needed.
  • Document all findings, plans, interventions, and client progress (outcomes) on designed forms and submit per Agency guidelines, every 48 hours.
  • Communicate plans and changes to the physician and to Nurse Manager and other Agency staff through the care plan, written progress notes, and participation in case conferences.
  • Guide and instruct the client in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning.
  • Advise, consult, and teach appropriate treatment procedures and supportive activities to clients, family members, and other Agency staff.
  • Design and/or order appropriate adaptive/assistive devices.
Requirements
  • Graduate of an accredited occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or is eligible for the National Registration Examination of the American Occupational Therapy Association.
  • Possesses current registration or license as required by state(s) of practice.
  • Minimum of two (2) year experience in a healthcare facility or equivalent experience.
  • Demonstrated good written and verbal communication skills interpersonal skills.
  • Licensed driver with automobile insurance in accordance with state/Agency requirements. Good driving record and reliable transportation.
  • Current CPR certification.
Working Conditions

Requires a full range of body motion including the handling and lifting of patients. Manual and finger dexterity and eye-hand coordination required. Occasionally will lift and carry items weighing up to 50 pounds or greater. Requires corrected vision and hearing to normal range. May require working under stressful conditions or working irregular hours. Involves exposure to communicable diseases. Requires a current driver's license and the ability to drive and access a patient's home without an assistive device.
